Facility Use Request

Please fill out this form if you would like to use our facility for a non-church event.

Please note there are two requirements to use our facility:
- A $50 usage fee
- A certificate of liability for your event (Available from your insurance company/agent. Please contact them for more info.)

Both are due in advance of your event date.
